- Attend a surgical weight loss seminar.

- Consider discussion with other people who have had the surgery or are considering surgical weight loss.
- Discuss your decision with your family physician.
Your primary care physician support is important.
- Contact your insurance provider regarding bariatric benefits.
- Schedule initial surgical evaluation with our office.

- Fill out the surgical application and information forms located in the side-bar of this site titled "Patient Information" and "Health History Questionnaire" (Links to those documents are also provided directly below.)
- Sign the release of information request form located in the side-bar of this site, or by using the link below. Please fill in the form and fax or mail it to our office so that we can acquire your medical records from your primary care physician.
